Unban is not granted just because you make a good and proper unban request, the format is important yes but another thing fair more important is the banhistory and you, like it or not, have crappy one :

I remark those two ban request becasue there you rageleft , left right after getting killed, and that's something I don't tolerate.
Yes, you are right when you suggest that I actually can’t know whether you left or had so tough luck for getting disconnected in such moment but don’t think you will fool me with skeptical arguments.
If you got disconnected in every situation not only you should have proved it but it would signify that your connection is not good enough for EDHLRs because even when you get disconnected you are ruining games.

I don’t accept your attitude of “you can’t know it so don’t judge and unban me” , as I’m giving you reasons and explanations for what I do I want you to explain me why I can’t think of rage leaving in those two reports, explain me the reasons why I shouldn’t do it.
