Denied.
1. You need separate ban request for every leaver
2. You need to put proper title of the thread. Every thread with title like this is automatically denied
3. Your screenshot is not acceptable. You need a ss with leaver has left the game msg, otherwise it will be denied
4. I suggest you read the guide how to post correct ban request
